He sees her headache as confirmation that nothing else is wrong. After he ceases instructing her to take breaths, he rubs the side of her arm for continued assurance. He relaxes the more he can see the panic subside from Ruby.

"Good. Focus on that. Just keep taking breaths and don't think about it too hard. You're doing great." He feels she's calm enough that she won't mind if he takes a closer look at her. He looks for any visible wounds on her before he lifts the bottom of her shirt high enough to see the old bullet wound underneath. Inspecting it, he sees nothing that causes any worry. He releases the fabric almost as soon as he lifts it. He knows right away she doesn't mean if saw any infection or not by her question and his response is to shake his head. "You're the same as I remember you--nothing has changed." Especially not in the way he feels about her.
